From page 305 of the Monster Manual (adjusted for the errata changes):

Alternate Form (Su): A creature with this special quality has the ability to assume one or more specific alternate forms. A true seeing spell or ability reveals the creature's natural form. A creature using alternate form reverts to its natural form when killed, but separated body parts retain their shape. A creature cannot use alternate form to take the form of a creature with a template. Assuming an alternate form results in the following changes to the creature:

-- The creature retains the type and subtype of its original form. It gains the size of its new form. If the new form has the aquatic subtype, the creature gains that subtype as well.

-- The creature loses the natural weapons, natural armor, and movement modes of its original form, as well as any extraordinary special attacks of its original form not derived from class levels (such as the barbarian's rage class feature).

-- The creature gains the natural weapons, natural armor, movement modes, and extraordinary special attacks of its new form.

-- The creature retains the special qualities of its original form. It does not gain any special qualities of its new form.

-- The creature retains the spell-like abilities and supernatural attacks of its old form (except for breath weapons and gaze attacks). It does not gain the spell-like abilities or supernatural attacks of its new form.

-- The creature gains the physical ability scores (Str, Dex, Con) of its new form. It retains the mental ability scores (Int, Wis, Cha) of its original form. Apply any changed physical ability score modifiers in all appropriate areas with one exception: the creature retains the hit points of its original form despite any change to its Constitution.

-- Except as described elsewhere, the creature retains all other game statistics of its original form, including (but not necessarily limited to) HD, hit points, skill ranks, feats, base attack bonus, and base save bonuses.

-- The creature retains any spellcasting ability it had in its original form, although it must be able to speak intelligibly to cast spells with verbal components and it must have humanlike hands to cast spells with somatic components.

-- The creature is effectively camouflaged as a creature of its new form, and it gains a +10 bonus on Disguise checks if it uses this ability to create a disguise.

From page 37 of the Player's Handbook (adjusted for errata):

Wild Shape (Su): At 5th level, a druid gains the ability to turn herself into any Small or Medium animal and back again once per day. Her options for new forms include all creatures with the animal type. This ability functions like the alternate form special ability, except as noted here. The effect lasts for 1 hour per druid level, or until she changes back. Changing form (to animal or back) is a standard action and doesn’t provoke an attack of opportunity. Each time you use wild shape, you regain hit points as if you had rested for a night.

Any gear worn or carried by the druid melds into the new form and becomes nonfunctional. When the druid reverts to her true form, any objects previously melded into the new form reappear in the same location on her body that they previously occupied and are once again functional. Any new items worn in the assumed form fall off and land at the druid's feet.

The form chosen must be that of an animal the druid is familiar with.

A druid loses her ability to speak while in animal form because she is limited to the sounds that a normal, untrained animal can make, but she can communicate normally with other animals of the same general grouping as her new form. (The normal sound a wild parrot makes is a squawk, so changing to this form does not permit speech.)

A druid can use this ability more times per day at 6th, 7th, 10th, 14th, and 18th level, as noted on Table: The Druid. In addition, she gains the ability to take the shape of a Large animal at 8th level, a Tiny animal at 11th level, and a Huge animal at 15th level. The new form’s Hit Dice can’t exceed the character’s druid level.

At 12th level, a druid becomes able to use wild shape to change into a plant creature with the same size restrictions as for animal forms. (A druid can’t use this ability to take the form of a plant that isn’t a creature.)

At 16th level, a druid becomes able to use wild shape to change into a Small, Medium, or Large elemental (air, earth, fire, or water) once per day. These elemental forms are in addition to her normal wild shape usage. In addition to the normal effects of wild shape, the druid gains all the elemental’s extraordinary, supernatural, and spell-like abilities. She also gains the elemental’s feats for as long as she maintains the wild shape, but she retains her own creature type.

At 18th level, a druid becomes able to assume elemental form twice per day, and at 20th level she can do so three times per day. At 20th level, a druid may use this wild shape ability to change into a Huge elemental.

化形(超自然)(Alternate Form):具有这种特殊能力的生物有能力呈现为一种或多种特定形态。真知术或类似能力能看穿该生物的原有形态。使用化形能力中的的生物在被杀死时会变回原有形态,但是分离的身体部分保持它们形状不变。生物不能使用化形变成带模板的生物。生物化形将会带来如下变化:
-该生物仍属于原有形态所属的种类和亚种。它获得新形态的体型。如果新形态有水栖亚种,该生物也会得到该亚种。
-该生物失去原有形态的天生武器、天生防御、移动方式以及不是来源于职业等级的(例如野蛮人的狂暴职业特性)属于特异能力的特殊攻击。
-该生物获得新形态的天生武器、天生防御、移动方式以及属于特异能力的特殊攻击。
-该生物保留原有形态的特性。它不获得新形态的任何特性。
-该生物保留旧形态的类法术能力和超自然攻击形式(除了喷吐武器和凝视攻击以外)。它不获得新形态的类法术能力和超自然攻击形式。
-该生物获得新形态的身体属性值(力量、敏捷、体质),维持原有形态的心智属性值(智力、感知、魅力)。任何身体属性值的改变都会带来相关领域修正值的改变,只有一个例外:即使体质改变,生物也保留原有形态的生命值。
-除了其他地方提到的,生物保留所有原有形态的游戏数据,包括(但是不限于)生命骰,生命值,技能级数,专长,基本攻击加值,基本豁免加值。
-该生物保留原有形态的所有施法能力,但它必须具有清晰的语言能力来施展需要言语成分的法术,也必须有象人一样的双手来施展需要姿态成分的法术。
-该生物能够非常有效地易容为新形态的生物,当它利用此能力进行易容时在易容检定上将获得+10的加值。
-该生物穿戴或携带的任何物品如果在新形态下不能穿戴或携带的话则会掉在它所在空间的地上。如果生物体型改变,任何能在新形态下继续穿戴或携带的物品也会变化体型以符合新形态。(非人形的生物不能穿戴为人形生物设计的盔甲,反之亦然。)如果掉落的话,装备恢复原本体型。

自然变身(超自然)(Wild Shape):5级开始,每天可以变身为小型或中型动物一次,然后再变回来。他可以选择变成任何动物型生物(见《怪物图鉴》)。此能力类似《怪物图鉴》中所述之「化形」特殊能力,但有如下几点不同:此效果持续小时数等于德鲁伊等级,或他自愿变回为止。改变身形(无论是变成动物或变回)是标准动作,但不会引发机会攻击。每次使用自然变身可以恢复相当于休息一整晚的生命值。
所有德鲁伊穿着或携带的物品都将与新型态融合,但没有功用。德鲁伊恢复原本型态后,融合的物品都将出现在原本的位置上并恢复功用。至于变身后穿戴的物品,回复原形时将掉落到该德鲁伊的脚边。
德鲁伊必须熟悉某个动物才能变成该动物。例如,若某德鲁伊从未离开过温带森林,便无法变成极地熊。
变成动物的德鲁伊无法说话,只能发出该动物正常未受训的自然声,但他可以与其他同种动物正常沟通(野生鹦鹉的声音是呱呱叫,所以即使变成鹦鹉也不能说话)。
德鲁伊达到6级、7级、10级、14级和18级的时候,每日可多使用一次自然变身,见表3-8。此外,8级开始,德鲁伊可以变身成大型动物。11级开始,可以变身成超小型动物。15级开始,可以变身成超大型动物。新身形的生命骰数不能超过人物的德鲁伊等级。例如,德鲁伊必须到12级才能变成凶暴熊(此大型生物的生命骰数至少为12HD),即使他在8级时就可以变成大型动物。
12级开始,德鲁伊可以变成植物类生物,例如蔓生怪,但仍须遵守体型限制(德鲁伊不能变成非生物的植物,例如树木或玫瑰丛)。
16级开始,德鲁伊可以变身成大型、中型或小型的土、水、火、风元素,每天一次,此元素身形不包含于自然变身的使用次数内。除了变身正常效果外,德鲁伊可以获得该元素的特异、超自然和法术型能力,而且也获得该元素的专长,但他的生物类型不变(大部分应是人形生物)。
18级开始,德鲁伊可以每天变身成元素2次。20级时为3次,而且可以变成超大型元素。
